Guidelines for Individual Project
Case Scenario
A multinational medical company operating several private clinics and a private hospital in Hong Kong. The company plans to develop a mobile app to improve service for patients with chronic conditions like hypertension and hypercholesterolemia. The app aims to enhance care coordination by allowing seamless data sharing across the health information systems of the clinics and the hospital. Through the app, patients will be able to access their laboratory results, medication records, and appointment schedules with reminders. Additionally, the app will provide educational materials on lifestyle modifications to support better disease management.
You are the nursing manager of the project development team of the company. Your team has reviewed successful mobile health apps in Hong Kong, including the Hospital Authority’s HA Go and the Paediatric Mobile Asthma App for Children from the KEC Smart Hospital Projects. Based on this research, you plan to select two to three suitable health information systems for integration with the new mobile app to ensure comprehensive and efficient patient data management.

Now, you are required to prepare a project proposal to introduce two to three IT systems with connection to mobile apps. In the proposal, the company require you to:
• explain the functions of two to three data management systems,
• illustrate the flow of information among the mentioned systems with the mobile apps, and
• demonstrate how the mentioned systems and mobile app contribute to holistic nursing care / healthcare.
Contents ofthe Project Proposal
Should be written in paragraph structure. General recommended format consisting of six elements as follows:
• Background
Provide an overview of the current situations, and briefly explain how the proposed project has the potential to achieve better outcomes.
• Executive Summary
Clearly interpret how the proposed project will enhance care coordination. Illustrate how information sharing between different clinics, the hospital via the data management systems and the mobile apps.
Explain how the mentioned IT systems with the mobile apps can enhance holistic nursing care / healthcare.
• Requirements
Outline the requirements for resources, tools, and project schedules throughout the project life cycle.
• Solution
Illustrate how you intend to approach and complete the project. It covers the project management steps, techniques, and skills needed to be productive and respond to interview requests.
• Authorization
Indicate who is the decision-maker for the project and the stakeholders authorized by the client to make the approval / sign-off decision.
• Appendix
Any information not inclusive in the actual proposal should be included in the appendix, for example, the company can use it to learn more about the project.
Evaluation Criteria
The student needs to show:
• Superior grasp of subject matter;
• Demonstrate excellent ability to apply the knowledge acquired and develop effective strategies to tackle the required task;
• Sufficient evidence of critical capacity and analytical ability;
• Good organization;
• Capacity to analyse and synthesize;
• Evidence of extensive knowledge base.

• Font style. Times New Roman
• Margins: 2.5 cm margin in each side of pages
• Spacing: Double-spaced
• Color: Black ink
• Pagination: Individual pages must be numbered - bottom of page and centre (digit only)
• Write your student ID (not your name) on every page (right top corner)
• Referencing style. APA style (7th edition)
Submission
• Due date: 3 December 2025 at 23:00
Soft copy (PDF file) of the assignment accompanied by the signed “Assignment Cover Page” must be uploaded to the course Turnitin Account / course Moodle at or before 23:00, 3 December 2025
• Weighting: 50% of the total marks of the course
• Length: No more than 2500 words. Words beyond the word count will not be marked.
Marks Deduction
For individual assessment, teachers may impose mark deductions for lateness, after considering particular student circumstances. A 5% per day deduction up to a maximum of 100% of the paper may be deducted.
Non-Submission of Work
Non-submission of work will be marked as zero.
